- Allow amdkfd to work with multiple kgd instances. This is in preparation for
  AMD's new open source kernel graphic driver (amdgpu), and for the new
  AMD APU, Carrizo.

- Convert timestamping to use 64bit time accessors

- Three other minor changes.
